Personal Support Worker

Personal Support Job Description

1. Empower people to maximize their independence, encourage and respect individual choices and preferences.

2. Support people to develop new relationships and to nurture existing relationships; support the development of interpersonal/social skills.

3. Encourage opportunities for personal growth and accomplishments; support people to achieve their individual goals and objectives

4. Provide supervision and support to ensure health and safety; support the development of safety skills; communicate medical concerns to supervisor; transport and accompany people to medical appointments as necessary

5. Provide supervision and support as need in the areas of personal hygiene, housekeeping, cooking, laundry, and problem solving.

6. Work cooperatively and positively with supervisor; follow through on plans and directions of supervisor.

7. Promote positive working relationship among staff; share exchange ideas openly and in a constructive manner; work cooperatively with others to ensure shift responsibilities are completed.

8. Attend and participate at staff meetings and in-service training; look for and take advantage of outside training opportunities to expand your knowledge.

9. Know and adhere to CC, LLC. policies and procedures including but not limited to Human Rights; residence Safety Plans, Employee Rules of Conduct, and Administrative On-Call.

10. Be respectful and courteous with people's families, friends, support coordinators and other circle of supports.

11. Maintain flexibility in order to respond to the evolving needs of the people we support; assume new and/or additional responsibilities as necessary and as assigned by management.

Supported Living Coach

Supported Living Coach


Supported living coaching services provide training and assistance, in a variety of activities, to support individuals who live in their own homes or apartments. These services may include assistance with locating appropriate housing, the acquisition, retention or improvement of skills related to activities of daily living such as personal hygiene and grooming, household chores, meal preparation, shopping, personal finances and the social and adaptive skills necessary to enable individuals to reside on their own. 

Supported living services mean the provision of supports necessary for an adult who has a developmental disability to establish, live in and maintain a household of their choosing in the community. This includes supported living coaching and other supports.
