Choose Connext Care as Your Provider

Let your Support Coordinator know that you would like to select Connext Care, LLC as your service provider


Step 3

Begin Services

Our team will meet with you to understand your goals and create a personalized support plan. Once services are approved, we begin providing the supports needed to help individuals grow in independence, confidence, and everyday life skills.
